We do accept most commercial insurance plans but always recommend contacting your insurance company to verify your coverage for in- or out-of-network coverage for visits and labs. Coverage can typically be verified by calling the number on the back of your insurance card or logging into your insurance account and using a 'Find a Provider' tool or similar. *Please see below for plans we are not presently able to accept as we are at capacity for these plans.
We have a 'Cash Pay' option! For those who do not have insurance or are not in contract we are happy to offer a 15% off discount on visits. This discount applies to the standard codes used for each visit type or procedure. Payment is due at time of service.
For Healthshare or Co-Op plans, we are unable to directly submit visits and codes for coverage with the exception of: Christian Medishare and Samaritans Purse. We can provide a detailed super bill for personal reimbursement at the time of checkout. These plans are processed as 'Cash Pay' and receive the 15% off! Payment is due at time of service.
MEDICARE
We are not accepting new Medicare Affiliated patients at this time.
Unfortunately Medicare insured patients are unable to be seen as a 'Cash Pay' Patient.

United HealthCare
We are not accepting United HealthCare Insurance for new or established patients at this time. United HealthCare insured patients (Not affiliated with Medicare) are eligible to elect to become a 'Cash Pay' patient at this time. A consent is required to defer commercial insurance as a 'Cash Pay' patient. Please note, insurance cannot be billed independently in this situation.
We will need the details from your insurance card to book any new patient visit. Please verify your coverage with your insurance plan prior to scheduling your visit as well as your insurance plan's preferred Lab for testing.

You will need your insurance card, the primary subscriber's date of birth, and a credit or debit card to book the appointment per our cancellation policy.
After scheduling, verify receipt of a login link to your online chart! If this did not make it into your email inbox, give us a call right away!
Login to your Online Chart through the link sent to your email to complete your intake. The intake is required and must be completed 7 days prior to your initial visit.

With our focus on Functional Medicine and looking at the whole person, your first appointment with us is when our providers get to know you as a patient and person. This is typically via Telehealth and is approximately 45 minutes long - there is no physical exam involved with this appointment. This appointment allows your provider to get to know you, your health history, genetics, family history, current and past treatments and symptoms you may be experiencing as well as your preferences when it comes to your care.
After getting to know you, your provider will order labs, typically a Full Functional Lab Panel, to look at hormones, blood counts, vitamin levels and other factors. At times additional or more comprehensive testing will be requested. Approximately 2 weeks after you have had your blood drawn, your provider will see you for your first in office visit for a physical examination, review labs, address any additional concerns and begin to establish a plan of action for your wellness. After this, we will see you at future visits as is appropriate in order to see you through to better health.
New Patient Pediatric appointments are considered 'Well Child Checks' and allow our providers to do a physical exam and learn about the pediatric patient's history. We know you may have many concerns and questions and we will be happy to address them! We may ask to schedule Follow-Ups or Telehealth appointments to thoroughly address concerns outside of the basic Well Child Check.
Any question or concern outside of a traditional Well Child Check will require appropriate codes as allowed by insurance and we may require a Follow-Up to allow our providers to dedicate additional time to researching, understanding, explaining, and addressing a concern.
